Ability to add logic inside of logic. For example: Being able to drill into different locations Country>State>City.

I would like form submissions to be able to be named automatically from the custom field data.

Would be great to get audio and video recording options for forms (so not just uploads), so users can record audio and video that gets attached to a form response straight away.

From Support Ticket: User wants to use a Form to create subtasks on a task, rather than a new task in the List

Use forms to update existing tasks fields. E.g in the inventory management template.. Use forms to CheckOut or CheckIn assets into the inventory using custom forms and update the value of stock in background.

It would be great to have sections in Forms just like in Google Forms and in Microsoft Forms. Also offer the ability to add logic to jump to a section.

From support ticket: I mistakenly created a form in the wrong list. I am supposed to give a demo to our team but need to move it to the correct list. How can I do this?

It would be great if there was an option when creating a form to send someone an email when they complete a form in ClickUp. I love how the form fields correspond to fields on a task, but we've had people ask if they can get a copy of the info they submit.

On clickup forms - when a client completes the form, if the "task name" exists (in my case "client name") we would like it update the task with other / new data collected from the form OR create a new Task / Client if they do not yet exist. I want it to be able to update existing clients or add new clients depending on the status. We have many clients who sponsor multiple events, and need to send or request different information dependent on the event they are sponsoring.
