Could you please remove the cookie notice from the form embed?
It means, when embedding on a site, a user gets 2 cookie notices - one form the site, and one form the form.
Is there even a reason to use cookies on forms?

Agreed! This is hugely disruptive to the site layout and confuses the end user, especially after they've already accepted cookies on the site.
Furthermore, clicking through to the cookie policy brings the user to "clickup.com/terms/cookies" which is doubly confusing, since the user will likely have no idea who or what ClickUp is, will have no idea how it relates to the site they're visiting, and may ultimately see it as a programming error on the website.
Finally, the code for the pop-up is causing the box to render the text in a nearly illegible manner (see attached), which likely does not conform to accessibility standards and reinforces the idea of a programming error.
In short, this is a terrible feature and should be removed!

I second this, we have our own cookie notice and this is a less than ideal user experience
